THE MISSION
The Sunnyside UP initiative is dedicated to improving health equity and awareness in the Sunnyside community by addressing systemic barriers, enhancing access to healthcare, and fostering collaboration among key stakeholders.
TARGET POPULATION
We focus on residents of Sunnyside, a predominantly Black community in Houston, TX, with particular emphasis on underserved populations including low-income families, individuals with chronic health conditions, and those facing barriers to accessing healthcare and essential resources.

Diabetes Prevention / Chronic Disease Management
Reduce the prevalence and impact of diabetes and other chronic diseases through education and management programs.
Maternal and Child Health
Improve health outcomes for mothers and children through targeted interventions.
